This research aims to critically analyze how community nursing contributes to managing chronic diseases in elderly individuals, and the challenges that both patients and nurses face in this context.

Objectives:
- To explore the methods used by community nurses to manage chronic disease symptoms.

Methodology:
- Qualitative Interviews with elderly patients who are living with chronic diseases, focusing on their experience with community nursing services.
- Interviews with Community Nurses will provide insight into their approach to managing chronic diseases, the tools and resources they use, and the challenges they encounter.
- Patient Health Records will be reviewed to assess hospitalization rates and chronic disease complications before and after receiving community nursing services.
